1. Nature of Workshop

The Institute provides an introduction to phlebotomy and EKG training workshops. Designed to teach basic blood draw skills and EKG lead placement. The training is not a state-licensed vocational program and does not award formal certification. 

2. Peer-to-Peer Practice & Informed Consent

As part of the hands-on training, students will be required to perform and receive venipuncture and capillary blood draws on each other under supervision. By agreeing to these terms, student gives full consent to both perform and receive blood draws during training. Student understands the inherent risks involved in blood collection procedures, including but not limited to bruising, hematoma, infection, nerve injury, or fainting.

3. Assumption of Risk & Release of Liability

Student voluntarily assumes all risks associated with participation in the workshop. Student releases, discharges, and holds harmless The Mays Institute of Phlebotomy, its owners, instructors, staff, agents, and affiliates from any and all claims, demands, actions, or liabilities arising out of or in connection with participation, including any injury, loss, or damage sustained. This release includes but is not limited to liability for negligence, accidents, or harm caused by other students during peer-to-peer practice.

4. Medical Conditions

Student affirms they are in good health and do not have any medical conditions, communicable diseases, or phobias that would interfere with giving or receiving phlebotomy procedures. It is the student's responsibility to disclose any concerns prior to participation.

5. Refund Policy

All payments are non-refundable due to limited seating and advance preparation for each session. Students may request to reschedule their class one time, provided that a 24-hour notice is given before the class start date. Rescheduling is subject to availability. 

6. Code of Conduct

Disruptive, unsafe, or inappropriate behavior will result in immediate dismissal without refund. Students are expected to treat all staff and peers with respect and follow all safety protocols as instructed.

7. Photographs & Media Release

The Institute may take photographs or video during classes for promotional or educational purposes. Student grants permission to be recorded unless a written opt-out request is submitted in advance.

8. Workshop Completion

A certificate of completion will be issued for successful completion of a workshop, but it is not an official license or certification. Students may consider additional education or pursue national certification from certifying organizations such as NAPTP to become Certified Phlebotomy Technicians or American Allied Health for Certified EKG Technician certificates.  

9. Compliance with Law

Student agrees not to misrepresent their completion of this workshop as certification or licensure. Student agrees to use skills responsibly and only in accordance with applicable local, state, and federal laws.

10. Indemnification

Student agrees to indemnify and hold harmless The Mays Institute of Phlebotomy from any claims, suits, costs, death or damages resulting from Student’s actions, conduct, or use of skills obtained.

11. Disputes

Student agrees that no disputes, claims, or legal proceedings shall be brought against The Mays Institute of Phlebotomy. By enrolling, the student fully waives the right to file any legal dispute, claim, or lawsuit related to workshop participation, injuries, or outcomes. This waiver includes but is not limited to disputes arising from instructional content, peer practice, or workshop operations.
